CROSSROADS VOLUNTEERS

The steps to becoming a volunteer are:

• Download the application (word, pdf), questionnaire (word, pdf), and background check form (word, pdf), or contact us and we will send them to you.  All forms can be emailed or faxed to us.  Our address is 6300 Chimney Rock, Houston, TX 77081 and our fax number is 713-295-2790.
• On the application, we will ask you for the names, addresses, & phone numbers of some references. Please write down the names of those you can count on to respond to a reference request.
• We also ask that you provide us with proof of your liability insurance and a copy of your drivers license.
• After we receive the application, we will begin the background check process. The background check will be done at the local, state, and national level.  You will also be asked to set a fingerprinting appointment through our office either at the time of training or you are welcome to stop by the office prior to being trained.  Once this appointment is set, you will be fingerprinted on the scheduled time at a third party location.
• You will be required to come to a training class.

Once these steps have been completed you will be assigned to a caseworker and a youth will be matched with you.
